Tobacco Cessation initiative
Our Tobacco Cessation initiative focuses on Population Health Risk Reduction Strategy, utilizes EHR to identify and refer tobacco users, offers free cessation resources for every patient using tobacco, promotes use of the Texas tobacco quit line, and impacts over 1.2 million Baylor Scott & White patients.
The tobacco status review begins when a provider documents the patient’s tobacco status in Epic. If the patient is identified as a tobacco user, a BestPractice Advisory appears advising the provider to open a SmartSet to refer the patient to the Texas Tobacco Quit Line.
If the patient is a current tobacco user, we ask if they’d like to be contacted by the Texas Tobacco Quit Line. If yes, the SmartSet process will begin. Once the SmartSet/referral is signed, the patient will be automatically referred to free tobacco cessation counseling and will receive free Nicotine Replacement Therapy (patch, gum, or lozenge).
Both aggregate and patient level data is fed back into the HER so impact and follow up can be easily evaluated.
